Fibroids are surprisingly common, with up to 80% of women developing some form of them by age 40. It's a staggering statistic, and for many, the thought of a hysterectomy and its side effects is concerning. After diving into some research, I found some interesting information worth sharing.

First, fibroids tend to decrease after menopause, which suggests a link to estrogen. When women experience pregnancy, use hormone replacement therapy (HRT), or take birth control pills, fibroids may enlarge, indicating a relationship with estrogen levels. Several risk factors for fibroids include obesity, type 2 diabetes, high blood pressure, and polycystic ovarian syndrome. Notably, Black women are three to nine times more likely to develop fibroids, with estrogen acting as the common thread. Estrogen is a growth hormone, driving the development of conditions like fibroids and endometriosis.

A fibroid is essentially a growth from the uterus, which is composed of muscle tissue. What sets a fibroid apart is its high collagen content, making it non-elastic and hard. Fibroids are also highly sensitive to estrogen and contain more estrogen receptors than the uterus itself. Additionally, they exhibit higher levels of an enzyme called aromatase, which converts androgens into estrogen, and exist in a low-oxygen, or hypoxic, environment. Furthermore, fibroids have increased levels of inflammation compared to the uterus.

The link between these factors is vitamin D deficiency. Research on vitamin D's role in fibroid treatment is mixed, partly due to inconsistent dosages in studies. The recommended daily allowance of 600-800 IU, focused on bone health, is insufficient for broader bodily needs, such as immune function and organ health. In higher doses, vitamin D has shown to inhibit fibroid growth, regulate other growth factors, and act as an aromatase inhibitor, reducing excess estrogen production. It also promotes apoptosis, the programmed death of fibroid cells, and has anti-inflammatory properties.

However, vitamin D struggles to be effective in hypoxic environments like those within fibroids, which may explain why higher dosages are necessary. Additionally, vitamin D helps modulate estrogen by supporting receptors that shrink tumor cells and blocking those that encourage growth. It also increases a liver hormone that lowers free estrogen levels in the body.

A major factor behind widespread vitamin D deficiency is melanin in the skin, which reduces vitamin D absorption. This particularly affects Black individuals, leading to lower levels and higher fibroid risks. Obesity is another contributor since vitamin D gets stored in fat cells, diluting its effectiveness. Conditions like type 2 diabetes and high blood pressure are also associated with low vitamin D levels, with supplementation shown to help manage these conditions.

Research on vitamin D's ability to shrink fibroids has shown some promise. For example, a 2013 study found that taking 25,000 IU of vitamin D3 weekly for 12 weeks significantly reduced fibroid size. A 2014 study showed similar results in African American women taking 50,000 IU every two weeks. Although more frequent dosing could potentially yield better outcomes, the findings indicate that vitamin D may be a viable approach.

Despite concerns over high vitamin D doses being toxic, evidence suggests it would take exceedingly large amounts over extended periods to reach dangerous levels. Side effects like high calcium can be managed with magnesium, vitamin K2, and proper hydration.

The data supports considering higher vitamin D intake, along with co-factors like magnesium and K2, as a potential strategy for shrinking fibroids.
